Important documents do not always happen in convenient moments. When an individual is in a hospital, senior living community, rehab center, hospice setting, or care facility, notarization requires patience, discretion, and a steady professional presence. This appointment provides mobile notary support for individuals, families, and caregivers who need eligible documents handled in a care-based setting. Common documents may include Power of Attorney forms, healthcare documents, affidavits, benefit forms, and other personal documents requiring notarization. Includes: Mobile service to a hospital, senior living, rehab, hospice, or care facility One signer One eligible document package Professional identity verification Signer awareness and willingness confirmation Proper notarial certificate completion Notary journal entry Up to 30 minutes of appointment time Appointments are structured to be calm, efficient, and handled with care. Please have the document completed, identification available, and all required parties present at the scheduled appointment time. Appointments begin at the scheduled time. Please provide the facility name, room number, parking instructions, and an onsite contact before the appointment. A valid, unexpired government-issued photo ID is required. Documents must be completed prior to the appointment with no blanks. The signer must be alert, aware, willing to sign, and able to communicate directly with the notary. Family members, caregivers, or facility staff cannot answer for the signer. The notary cannot provide legal advice, assist with document preparation, or proceed if the signer appears confused, unwilling, unaware, under pressure, or unable to communicate directly. Witness coordination is available as an add-on for $60 per witness and must be requested and confirmed in advance. Additional fees may apply for additional notarial acts, signers, documents, extended time, printing, travel outside the standard service area, parking, or facility access fees. Payment is due at booking to secure your appointment. Late cancellations, no-shows, or appointments that cannot be completed due to missing documents, unavailable parties, incomplete forms, ID issues, facility access issues, signer awareness concerns, or unconfirmed witness requirements may still be subject to the full service fee.
