Service Description

Planning ahead with a trust provides greater control, privacy, and protection for your estate while helping your loved ones avoid the probate process. This package is designed for couples completing separate individual trust-based estate plans during the same appointment. Each signer establishes their own trust and supporting documents. • Revocable Living Trust – Establishes a trust structure to manage and distribute assets according to your instructions. • Certificate of Trust – Verifies the existence of the trust without disclosing private details. • Property Transfer Deed (One Property) – Transfers eligible real estate into the trust. • Pour-Over Will – Directs any remaining assets into the trust upon death. • Assignment of Property to Trust – Transfers designated personal property into the trust. • Medical Power of Attorney – Authorizes healthcare decision-making if needed. • Financial Power of Attorney – Grants authority to manage financial matters. • HIPAA Authorization & Release – Allows access to medical records for informed decisions. Actual documents and notarization requirements may vary depending on your estate plan. This Service Includes: • One grantor/trustor • Trust-based estate signing appointment • Mobile travel to designated service location • Appointment time up to 60 minutes Appointment Timing & Grace Period Please ensure the signer, identification, witnesses, and documents are ready at the scheduled appointment time. Appointments exceeding the included 60-minute service window may require additional time to complete. Additional Fees May Apply For: • Additional notarizations beyond the included amount • Additional deeds beyond the included property • Professional Witness • Additional printed document sets • Additional appointment time beyond the included service window Important Booking Requirements: • The signer must present valid, unexpired government-issued photo ID • Documents must be fully completed prior to the appointment (no blanks) • The notary cannot provide legal advice or assist with document preparation • The signer must be alert, aware, and willing to sign at the time of service • Witness requirements must be confirmed prior to the appointment Deposit Requirement A $50 deposit is required at the time of booking to secure your appointment. The remaining balance is due at the time of service. Deposits are applied toward your total service fee and are non-refundable in the event of late cancellations or no-shows.
