Service Description This appointment is for couples signing prepared trust, will, Power of Attorney, healthcare directive, and related planning documents during the same appointment. Each signer must provide their own completed documents and should not sign until the notary appointment. The Notary Atelier Co. provides notary support only. We do not draft documents, prepare estate plans, select forms, explain legal terms, or provide legal advice. Clients should consult an attorney or document preparer with questions about document content, legal effect, or required forms. This Service Includes: • Two signers • Notary appointment for prepared trust, will, Power of Attorney, healthcare, or related planning documents • Mobile travel to the designated service location or private in-office appointment, if selected • Identity verification for each signer • Notarial certificate completion where required • Journal entries as required by Illinois notary law • Appointment time up to 90 minutes Important Booking Requirements: • Both signers must present valid, unexpired government-issued photo ID • Documents must be fully completed before the appointment, with no blank sections • Documents must not be signed before the notary appointment • Both signers must be alert, aware, and willing to sign at the time of service • Witness requirements must be confirmed before the appointment • If witnesses are required, clients may provide their own eligible witnesses or request professional witness coordination when available Additional Fees May Apply For: • Additional notarizations beyond the included service scope • Professional witness coordination • Additional printed document sets • Additional appointment time beyond the included 90-minute service window • Extended travel, parking, or special location requirements Payment Requirement: Payment is due at the time of booking or at the time of appointment, depending on the booking method. Appointment fees are based on service type, location, document needs, and any requested add-ons.
