This package is designed for individuals completing a standard will-based estate plan and the essential documents that typically accompany it. This package typically includes: Last Will and Testament – Documents asset distribution and guardianship preferences, if applicable. Self-Proving Affidavit – Allows the will to be accepted in probate without requiring witness testimony. Memo for Personal Property – Allows you to designate how sentimental and personal items should be passed on. Medical Power of Attorney – Authorizes a trusted individual to make healthcare decisions if you become unable to do so. Financial Power of Attorney – Grants authority to manage financial matters in the event of incapacity. HIPAA Authorization & Release – Allows designated agents access to medical information for decision-making. Actual documents and notarization requirements may vary depending on your estate plan. This Service Includes: One testator Will-based estate plan signing appointment Mobile travel to designated service location Appointment time up to 60 minutes Appointment Timing & Grace Period Appointments include a 15-minute grace period for arrival and preparation. Please ensure the signer, identification, witnesses (if required), and documents are ready at the scheduled appointment time. Appointments exceeding the included 60-minute service window may require additional time to complete. Additional Fees May Apply For: Additional notarizations beyond the included amount Witness coordination (if required by your documents) Additional printed document sets Additional appointment time beyond the included service window Important Booking Requirements: The signer must present valid, unexpired government-issued photo ID Documents must be fully completed prior to the appointment (no blanks) The notary cannot provide legal advice or assist with document preparation The signer must be alert, aware, and willing to sign at the time of service Witness requirements must be confirmed prior to the appointment Deposit Requirement A $50 deposit is required at the time of booking to secure your appointment. The remaining balance is due at the time of service. Deposits are applied toward your total service fee and are non-refundable in the event of late cancellations or no-shows.
